Medication Administration
EXACT Sport LLC ("EXACT") will not administer medications to event participants of any age. EXACT will not be held responsible for housing/storing medication(s). Parent(s)/Legal Guardian(s) and event participants will be held responsible for administering and housing/storing medication(s) in a discrete place during the event and/or relying on certified medical practitioners as needed.
Immunizations
As of this date, has the athlete received all immunizations required under state law? Please contact your family's medical professional practitioner to determine the vaccinations and immunizations appropriate. Athletes who do not comply will not be allowed to participate in the event.
COVID-19
EXACT puts in place preventative measures to reduce the spread of COVID-19; however, EXACT cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending an EXACT event could increase your risk and your child(ren)'s risk of contracting COVID-19. By participating in this event, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19.
Medical Treatment Consent & Indemnification
My child has permission to engage in all prescribed event activities. The information provided on this form is accurate to the best of my knowledge. I have indicated any special health conditions, including required medication and activity limitations which should be known to the event staff and medical personnel. I am aware of and accept the risk inherent in the program activity. I give consent in advance for medical treatment at an appropriate facility in case of illness or injury. I agree to indemnify EXACT and its employees and agents for any claim which may hereafter be presented by our (my) son/daughter as a result of any such injuries.
Event Rules & Liability (residential events)
Event Rules & Liability: Event participants are expected to behave lawfully and follow all rules and guidelines at all times. Failure to do so can result in immediate termination from the program without refund. Event participants are responsible for issuing a credit card for hotel incidentals at the time of check-in. Event participants will be fully responsible for any incidental charges incurred at the camp hotel, including but not limited to property damage of any camp premises (including hotel), theft of hotel property, damage to personal property, or any hotel expense.
Media Release
I grant EXACT the right to use photographs and video taken of my child, myself, and members of my family while at EXACT Camps for any lawful purpose.
Traumatic Brain Injury (TBI) Awareness & Return-to-Play Waiver
Traumatic Brain Injury ("TBI"), including concussions, is a serious medical condition that can occur as a result of participation in athletic activities. TBIs may result from a blow, bump, or jolt to the head or body and can have short-term or long-term health consequences.

TBI Awareness & Safety Program Acknowledgment. I acknowledge that EXACT Sport LLC, Student Athlete Scholars LLC, and/or All State Game LLC (collectively referred to as "EXACT") has provided information regarding the nature of traumatic brain injuries, the risks associated with athletic participation, and the common signs and symptoms of TBI, which may include (but are not limited to): headache, dizziness, confusion, nausea, sensitivity to light or noise, memory issues, or changes in behavior. I understand the importance of immediately reporting current or suspected head injury, concussion, or symptoms of traumatic brain injury to event staff or medical personnel (when available).
Prior Traumatic Brain Injury Disclosure. I understand and agree that any athlete with a prior or suspected traumatic brain injury must receive written medical clearance from a licensed healthcare provider trained in concussion/TBI management, as permitted by applicable state law before participating in any EXACT event activities. I further understand that failure to obtain and provide such written clearance may result in the athlete being withheld from participation. I understand and agree that if a concussion or TBI is suspected (based on the athlete's report, parent report, staff observation, or medical evaluation (when available)), the athlete will be removed from participation promptly and will not return to activity the same day.
This acknowledgment is not medical advice and does not create a medical provider-patient relationship. TBIs/concussions can be difficult to detect, and EXACT does not guarantee that injuries will be prevented or identified. EXACT does not diagnose TBIs/concussions and relies on the athlete's licensed healthcare provider's written clearance for return-to-participation decisions. |
